Founded in 2003, HOPE Community Medicine is a nonprofit Federally Qualified Health Center serving patients and families across Deep East Texas. Our mission is simple: provide high-quality, affordable healthcare and make sure every person has access to care, regardless of their ability to pay.
HOPE offers comprehensive healthcare services, including primary care, behavioral health, psychiatry, women’s health, OBGYN, pediatrics, pharmacy services, and community resources. From routine checkups to ongoing care, our team is here to support the health and well-being of individuals and families at every stage of life.
What started as a small community clinic has grown into a multi-site healthcare organization rooted in compassion, faith, and service. We are proud to care for the communities we call home while continuing to expand access, reduce barriers, and improve health outcomes across rural East Texas.

HOPE Community Medicine accepts most major insurance plans, including Medicaid and Medicare. We also welcome self-pay patients. If you are unsure about your coverage, our team can help verify your insurance and answer questions before your visit.
Yes. HOPE is here to serve the community, regardless of insurance status. We offer a sliding fee program for eligible patients based on household income and family size, helping make healthcare more affordable.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) and an FTCA deemed facility.
This health center receives HHS funding and has Federal Public Health Services (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
